Free fall sports physicals for Harrisburg
School District students only.

TIME: 2:30 pm to 4:30 pm
LOCATION: Bloom Building @ UPMC Community General Osteopathic Hospital
4310 Londonderry Road, Suite B, Harrisburg, PA 17109

• Arrive 15 minutes early. If students arrive late, they will not be seen by the doctor.
A parent must be present for the physical and masks are required.
• Complete all information online before arriving at the doctors office. Bring with you section 5&6 of the PIAA CIPPE form for the doctor to review and complete.

• All physicals must be dated AFTER May 15th Physicals must be completed prior to the start of the season.
